• Birth Certificate
• Social Security Number
• Immunization Records - see Health Services for current requirements and forms
• Physical Examination - All students entering kindergarten, seventh grade, or transfer students from out of state are required to have a physical examination by a licensed physician before entering these grades.  The physical exam must be within six months prior to the beginning of school.  Parents who object to their children having an exam may forego this requirement by signing a waiver form, available in the office of the building principal.
