Parent(s)/Guardian(s) of 4th Grade Students
4th GRADE BEGINNING BAND TEST DRIVE 2015
Last year we started beginning band during the summer and we will be doing the same this year.  This helped facilitate getting the schedule set right from the beginning of school for 5th grade band students.
With that in mind, I plan to have our band test drive for beginning band right after the school year.  I would like to do that the week of May 26-May 29.  I will have you come in for an appointment with me during that week.  At that time I will let the students try out instruments and choose the best fit for each student to play in 5th grade band.  I will then begin their lessons during the month of June, beginning June 8th and running 5 weeks until July 10th.  I would ask that each student come in once a week for that month for a 30 minute lesson, (If they want to come with a friend that plays the same instrument that would be OK).  That way we would get started in the summer and be all ready to go in August when school starts.
PLEASE CALL AND VISIT WITH ME IF YOU HAVE ANY QUESTIONS AT ALL ABOUT THE PROCESS.  DON’T FEEL OBLIGATED IN ANY WAY. harley.mohlman@bbps.org 402-310-6322
PERHAPS YOUR STUDENT WOULD LIKE TO TRY OUT AN INSTRUMENT, BUT ISN’T SURE ABOUT MAKING THE COMMITMENT.  THIS IS A GOOD TIME TO DISCUSS THAT SITUATION.

School Sport Physicals

Central Nebraska Medical Clinic – May 19, 2015 from 4:00 – 6:30pm
You may pick up the sports form in the Jr. High and High School offices. We will also have them available at Custer Elementary for the 6th grade students.  The clinics will get the forms to the school as long as the parents have signed the form.  It is not necessary for a parent/guardian to be present as long as they have signed the sports physical form prior to the exam. The appointments are walk in only and the cost will be $30.00 to be paid at the time of the exam unless you want your insurance billed. Please note that at the time of your arrival for the physical.

Broken Bow Clinic – Broken Bow Clinic will not host a specific day for physicals this year.  You may bring in the sports physical form to be filled out for a physical at any time after the 1st of May if you want insurance billed. If you want to pay the $30.00 cash for a sports physical then you can make an appointment anytime during the month of May and you will need to pay the $30.00 at the time of the physical.  The $30.00 fee is for the month of May only. You must make an appointment at this clinic and let them know if you want it billed to your insurance or if you will be paying cash. Again the parent or guardian does not have to be present as long as the form is signed prior to the physical.

Parents need to sign the form in both highlighted places in order for the clinic to release the physicals to the school district.  The clinics will no longer be doing urine tests.  7th graders need to make an appointment any time after May 1st because no 7th grade immunizations will be given on the 19th at CNMC.  Required immunization for 7th grade is Tdap.  If you have any questions please feel free to contact the school nurse at 870-3078.
